This presentation will examine the strategies and use of evidence-based transition practices for transition planning, discuss the role of transition assessments, and explore activities and options to prepare students for postsecondary education and/or training, competitive employment, and living and participating in communities. This session will also offer transition stakeholders a glimpse into several research and demonstration projects that have been implemented across Virginia to improve career and college readiness for youth and young adults with disabilities.
